UnitedHealthcare Covers Addiction Treatment

UnitedHealthcare (UHC) is the largest commercial health insurer in the United States, covering over 50 million members. All UHC plans are required to cover substance abuse treatment under the Affordable Care Act and the Mental Health Parity and Addiction Equity Act.

Your UHC plan should cover:

  • Medical detoxification
  • Inpatient/residential treatment
  • Partial hospitalization programs (PHP)
  • Intensive outpatient programs (IOP)
  • Individual and group therapy
  • Medication-assisted treatment (MAT)

Coverage details differ between UHC commercial plans, employer-sponsored plans, and UHC Community Plans (Medicaid managed care).

How UHC Plans Work for Rehab

UHC Commercial and employer plans

Most commercial UHC plans cover rehab with the following structure:

  • In-network coverage: 70% to 90% of costs after deductible. Many employer plans cover even more.
  • Out-of-network coverage: 50% to 60% of “usual and customary” charges. You are responsible for any balance above what UHC considers reasonable.
  • Prior authorization required for inpatient stays, PHP, and some IOP programs.

UHC Community Plans (Medicaid)

In states where UHC manages Medicaid benefits, coverage for substance abuse treatment is typically more comprehensive with lower out-of-pocket costs. Medicaid managed care plans through UHC usually cover:

  • Medical detox with no copay
  • Inpatient rehab for clinically determined lengths of stay
  • Outpatient treatment including IOP with minimal cost-sharing
  • MAT medications including Suboxone, methadone, and naltrexone

Verifying Your UHC Benefits

Follow these steps before entering treatment:

  • Step 1. Call the number on your UHC card. Ask for the behavioral health team.
  • Step 2. Provide your member ID and ask about substance abuse treatment benefits.
  • Step 3. Ask specifically about: deductible (has any been met this year?), coinsurance percentages for inpatient and outpatient care, any day or visit limits, prior authorization requirements, and your out-of-pocket maximum.
  • Step 4. Use myuhc.com or the UHC app to find in-network behavioral health providers near you.

Write down the reference number for your call. If there is a dispute later, this number documents what you were told about your benefits.

UHC’s Utilization Review Process

UHC uses a process called utilization review to determine how long your treatment will be covered. Here is how it works:

  • Your treatment facility submits a request for a specific number of days
  • A UHC clinical reviewer evaluates whether continued treatment is medically necessary
  • UHC approves, modifies, or denies the request
  • Reviews typically happen every 7 to 14 days during inpatient stays

This means UHC may approve 7 days initially and then extend coverage in increments as your treatment team demonstrates ongoing medical necessity. Your facility’s utilization review team handles this process, but you should be aware of it.

What If UHC Denies Your Claim?

Common reasons for denial include:

  • UHC determined the level of care is not medically necessary (for example, recommending outpatient instead of inpatient)
  • Prior authorization was not obtained
  • The treatment facility is out-of-network
  • UHC believes you can be safely treated at a lower level of care

Your appeal options

  • Peer-to-peer review. Your doctor speaks directly with a UHC medical director. This is often the fastest path to overturning a denial.
  • Internal appeal. Submit within 180 days. UHC has 30 days to respond for non-urgent cases, 72 hours for urgent cases.
  • External review. If the internal appeal fails, request an independent external review. This costs nothing and is binding on UHC.
  • State insurance department. File a complaint with your state’s department of insurance if you believe UHC violated parity laws.
